ATHLETE PAPERWORK AND INSTRUCTIONS

Dear Parents/Guardians and Student-Athletes:

On behalf of the Intercollegiate Athletic Department at the University of Nebraska at Kearney, the Sports Medicine staff would like to welcome you and hope you enjoy an injury free career. The following information is EXTREMELY IMPORTANT! We need your time and cooperation to carefully and accurately complete all the requested information and forms.

THIS INFORMATION MUST BE COMPLETED AND RETURNED BEFORE YOU WILL BE PERMITTED TO PARTICIPATE IN PRACTICE WITH YOUR RESPECTIVE TEAM. PLEASE MAKE SURE ALL PAPERWORK IS FILLED OUT COMPLETELY BEFORE SUBMITTING TO THE ATHLETIC DEPARTMENT.

All pre-participation paperwork will be completed online at www.swol123.net. You must fully complete every form/tab before you will be cleared and allowed to participate in any sports related activity.

In addition each athlete is required to pay a $50 athletic participation fee to be paid after July 1st

IT IS MANDATORY THAT WE HAVE ALL FORMS COMPLETELY FILLED OUT AND ON FILE WITH THE ATHLETIC DEPARTMENT TRAINING ROOM. ANY UNK ATHLETE WHO HAS NOT TURNED IN ALL OF THE REQUIRED INFORMATION WILL NOT BE ABLE TO PARTICIPATE IN ANY ATHLETIC RELATED ACTIVITY UNTIL ALL INFORMATION HAS BEEN CORRECTLY FILLED OUT AND SUBMITTED.
